Physician Assistant students must complete the Immunization Form and submit it to Student Health by April 3. The immunization form is to be input and uploaded into the Point and Click portal. Please call the Student Health office if you have any questions or concerns. Non-compliance may prevent you from beginning classes.
The required immunization form is to be completed and signed by your health care provider. Make sure to include copies of the lab reports displaying a quantitative titer and IGRA result. Information must be recorded on Penn State forms; “See attached” for immunization records will not be accepted. All required immunizations and titers must be completed and submitted to Student Health by April 3 of your incoming year.
