Accepted Physician Assistant Students

Physician Assistant students must complete the Immunization and Infectious Disease Form and Preregistration Form and submit to Student Health by April 3. These forms must be submitted in person, via fax or mail. You may not send them via email. Please call the Student Health office if you have any questions or concerns. Non-compliance may prevent you from beginning classes.

The required forms are:

  1. Immunization form completed and signed by your health care provider. Make sure to include copies of the lab reports displaying a quantitative titer and IGRA results. Information must be recorded on Penn State forms. “See attached” for immunization records will not be accepted. All required immunizations and titers must be completed and submitted to Student Health by April 3 of your incoming year. If you have started but not yet completed your required immunizations due to recommended time intervals for vaccines, remaining vaccinations will be arranged by our Student Health office after your program start date. If you have questions about the immunization requirements, you may call our office at 717-531-5998.
  2. Pre-registration form completed with a copy of your current insurance card attached. If you do not have insurance, you will learn more about the insurances available to you as a student at orientation.
  3. OHSA Respirator Medical Evaluation Questionnaire completed. All students are required to complete this questionnaire at least once during their tenure at the College of Medicine.

Fees will be incurred for not completing these requirements before April 3. The fees are as follows:

  • After April 3: $50
  • After April 17: $100
  • After May 1: $150

Students accepted after March 31 will have five weeks from their acceptance date to complete their requirements before incurring the $50 fee. The fee increases to $100 after six weeks, and to $150 after eight weeks.

We will only accept forms and the accompanying documentation by mail or fax. Emailed forms and documentation will not be accepted.
