Graduate students must have an Infectious Disease Form completed by a primary care provider or a school nurse. The Form must be downloaded and completed and is to be input and uploaded into the Point and Click student health portal.
All immunizations must be completed before students arrive on campus. In addition to returning the form to the Student Health Office, students must directly enter the vaccination records information into the student health portal and must then upload the documentation into the portal.
